REGISTRATION FOR 2014-15 SCHOOL YEAR Worth County School System- Sylvester, Georgia Information required on any student transferring from another school system that will attend Worth County Middle (grades 6th- 8th) for the 2014-15 school term is listed below. Proof of Worth County Residency (only 1 document from the list is needed): Worth County Tax receipt or Ad Valorem tax receipt, Current Utility Bill, Current Lease Agreement, Warranty Deed, Current Telephone bill or Cable bill (not a Cell phone bill), Worth County Tag receipt or Current Rent receipt (must be notarized and include parent/guardian name & physical address) Proof of Guardianship/Custody: Parent’s ID, guardianship papers or a divorce decree. Birth Certificate, EED, Social Security Number, Immunization (Form 3231) All new students must be up to date with shots. (See requirements below). Report Card & Test scores (CRCT, ITBS, etc.) from the 2013-14 school term. Name, address, telephone & fax number of the last school attended. Registration Dates: Tuesday, July 15, 2014 Wednesday, July 16, 2014 Thursday, July 17, 2014 8:30 a.m. – 11:45 a.m. 1:30 p.m. – 3:45 p.m. Beginning July 1, 2014, all children born on or after January 1, 2002 who are attending seventh grade, and students who are newly enrolled in Georgia schools in grades eight through 12, must have received one dose of Tdap vaccine and one dose of adolescent meningococcal (meningitis) disease vaccine. Proof of Tdap Immunization & Meningitis Vaccination must be documented on the Georgia Immunization Certificate (Form 3231).
